partnerships with governmental agencies and innovative programs
like Project SAFE (PSafe) and Prevention Come be part of a team
dedicated to empowering adolescents, adults and families facing
mental and Aftercare (P&A), we foster long-term skills and
support systems, enhancing the well-being of individuals and
families across all stages of life. Position Details: Full-Time
Location: Sherman Oaks Key Responsibilities Conduct phone screening
and outreach services with assigned clients and families as
required by the program’s structure Provide supportive assistance
to the referred client family in the assessment strengths,
protective factors and determination of needs Actively participate
in multidisciplinary team conferences, meetings Meet with assigned
clients twice per month or as needed to provide community supports,
assistance with medication, (i.e., reminder calls, pharmacy calls,
health insurance and resource applications including food stamps
transportation, etc.), and applications for disability when
warranted Provide support to clients and families regarding
education and employment, as needed, by collaborating with
families’ P&A multidisciplinary team including subcontractors
Assist with securing adequate living arrangements, vocational
activities, schooling, supportive services, medical and mental
health services Assist clients and their families in linking with
other service agencies and subcontractors, as necessary Attend
outside agency meetings with community liaisons as assigned
Collaborate with P&A’s Therapist Primary Designee and the
County P&A Program Coordinator regarding coordination of
schedules Qualifications Bachelor’s degree in related field
(psychology, sociology, social work, special education or human
services) strongly preferred Minimum 2 years’ experience working
with at risk children and TAY-aged clients and their families
Strong social and communication skills, verbal and written to work
with and within the multidisciplinary treatment team and ability to
flex work schedule to accommodate client needs and P&A Program
community engagement activities Ability to collaborate with
community partners and develop, coordinate and oversee supportive
educational, employment, mental health and other resources in
accordance with the P&A Program requirements Current California
Driver's License and a driving record acceptable to the Agency's
insurance carrier, if required by job duties Bilingual Spanish

range represents the low and high end of the salary someone in this
role may earn as an employee of Help Group. Salaries will vary
based on various factors, including but not limited to,
professional and academic experience, certification, training,
responsibilities of the position, and other business and
organizational needs. The range listed is just one component of our
total compensation package for employees. Salary decisions are
dependent on the circumstances of each hire. Help Group reserves
