Job Description:
A company is looking for a Care Management Associate for the
Engagement Hub, working from home in Florida.
Key Responsibilities
Conduct telephonic outreach to engage and enroll eligible Medicaid
members
Facilitate case assignment and connect members to care managers
through warm transfers
Maintain accurate documentation and adhere to compliance and
confidentiality standards
Required Qualifications
2 years of experience in the healthcare field preferred
Proficiency in Microsoft Word, Outlook, and Excel
Familiarity with basic medical terminology and care management
concepts preferred
Flexibility to work occasional nights and weekends
Must reside in Florida
